Death in these patients usually occurs because of pulmonary infarction or progressive deterioration of the hemodynamics with a fall in peripheral resistance and worsening cyanosis despite all pharmacologic interventions. It is not possible for the unborn baby to oxygenate its own blood. AVSD is associated with atrioventricular AV valve defects — tricuspid valve of the right side or mitral valve of the left side. This leads to a structural abnormality in the heart or vessels which depending on the type and extent may case mild to severe symptoms or can even be life threatening.

The ASD leads to a left-right shunt and to an overload for the right ventricle with recirculation through the lungs. Prenatally, even a large defect is not disadvantageous because roughly the same pressure is present in both ventricles. Yet, a PDA is said to cause a right-to-left shunt. Small defects lead to no detriment and especially those in the muscular septum sometimes close spontaneously.

Cardiology

The two circulations are placed in series with each other. The same volume of blood first makes its way through the systemic circulation, then the pulmonary circulation, then back to the systemic circulation, and so on. As a consequence, the pulmonary circulation carries not only the blood that entered the right atrium and right ventricle through the superior and inferior vena cava, but also the additional blood entering through a VSD, ASD, AVSD or PDA. If the shunt is significant, there is progressive damage to the pulmonary vasculature and gradual development of irreversible pulmonary hypertension. The pressure in the pulmonary circuit may ultimately exceed the systemic pressure causing reversal of blood flow from the right side of the circulation to the left Eisenmenger syndrome. This may take as short as 1-2 years in a large VSD, AVSD or PDA or as long as a few decades as in ASD. In large VSD and PDA, the direction and magnitude of the shunt depends on the relative resistance in the pulmonary and systemic circuits. In ASD, the magnitude of the shunt depends largely on relative ventricular compliance elasticity. AVSD is a mix between ASD and VSD depending on the predominant lesion. The defect size is determined by comparing its diameter with the aortic annulus.

Catheter-based sin of the structure was first performed in 1971. Sometimes Ibuprofen can be used with less side-effects. Although a left ductus arteriosus is a normal structure during normal fetal development, the presence of a right ductus arteriosus is usually associated with other congenital abnormalities of the glad system, most typically involving the aortic arch or conotruncal development. Pulmonary hypertension is an infrequent, late complication because increase VOLUME flow through pulmonary artery. At this stage, the PVR is irreversible, closure of the patent ductus arteriosus PDA is contraindicated, and lung transplantation may be the only hope for north-term survival.
